There's a stock letter at http://www.nhsconfidentiality.org/optoutletter - 
it'll even put in your name and address etc ready for you to print out 
(without storing them:-) ). I put one in the post to the surgery this 
afternoon.

Not sure about hospital records - the BBC story implied it was a GP thing, 
but perhaps we should write to the hospital(s) too.
