[milton-chat] medical records

I don't want my medical records put on the government's 
central computer where they will be available to people in 
other government departmets such as social services and 
police.  Unfortunately I don't know the terminology, so can 
someone suggest suitable wording for a letter to Milton 
Surgery ?  What can I do about hospital records ?
